Membership Playbook:
Simple, Transparent Savings
- Membership is for a full 12 months from start date.
- Payment is due for the full year at the start of membership.
- We recommend appointments be 6 months apart, but do not require it.
Exclusions: Membership discount does not cover removable appliances (such as mouthguard and night guards), sedation fees (with our office and with outside companies), any fees associated with treatment at Cincinnati Children’s Hospital, the sale of any products (toothpaste, toothbrushes, etc.).
* This dental plan is not insurance. No refunds or cancellations. Cannot be combined with insurance plans or discount programs. You cannot use CareCredit or Cherry to pay for your membership plan. Rates are subject to change at renewal. Membership must be current to receive included or discounted services.

Ages 3–13 | $334/year

Teen
Ages 14+ | $394/year
